PRE-ARRIVAL / ARRIVAL

A) Housing      1. Arrange temporary housing if necessary

2. Arrange furnishings and basic necessities

3. Arrange permanent housing

 4. Call City Zoning Authority (428-6521)

5. Turn on R G&E utilities under refugee’s name

6. Turn on telephone (when Social Security #s are received)

B) Reception Meet refugee family at airport. Arrange for sufficient transportation for family and luggage

C) Initial Food: Arrange for some food or a meal to be ready for refugee family upon their arrival from airport

1) Emergency Food Stamp DSS 111 Westfall Rd. No appointment necessary.

•       Bring ONLY DSS Application, Request for Exp. Food Stamps Letter from CFC and family’s 1-94 cards.

•       All members in the case 18 and older must attend interview.

 

2) Social Security Cards Federal Bldg. 100 State St. or 1100 Long Pond Rd.

•       Bring completed Application, Request for Soc. Sec. Cards Letter from CFC

and family’s 1-94 cards

•       All members in case 14 and older must attend

•       Obtain receipts and return a copy of each to Case Manager

 

3) TB Clinic 111 Westfall Rd. 274-6161 (Monday only between’ 9am & 11am) or Bullshead Plaza

855 W. Main St. 464-5903 (Monday, Tuesday or Wednesday 8am to 2pm or Friday 8am to 11am)

•       ALL members in the case must go within one week of entering US.

•       Bring I-94s

•       An interpreter must accompany refugees

•       Clients must return to clinic 2 days later to have their results read.

•       Obtain TB Verification Receipts and return copies to Case Manager

 

4) Immunization Clinic        111 Westfall Rd. 274-6161 (Wednesday only between 12pm & 7pm)

•       All school age children (5 thru 17) must be immunized

•       Bring I-94s and any overseas medical records

•      At least one parent and an interpreter must accompany refugee children

•       Obtain Verification of Immunization Form

 

5) Health Clinic  Family Medicine Center (885 South Ave. 442-7470) or

Jordan Health Center (82 Holland St. 423-5800) or Brown Square Clinic (175 Lyell Ave. 254-6480)

or Family Health Associates (301 Exchange Blvd. 262-3020)

•       Call to make appointment and be prepared to provide birth dates

•       All members in case must go to health clinic within 30 days of arrival

•       An interpreter must accompany refugees

•       Bring I-94s and any overseas medical records

6) CFC Orientation Interview                                87 North Clinton Ave. 1st Floor (Within one week of arrival)

•       All members in the case 18 & older and sponsor must attend.

•      Bring all documents in IOM bag including I-94s, airfare promissory notes, IOM health reports, passports, related immigration documents and

any receipts received from DSS, SS office or Medical/TB clinics.

 

  7) Placement Center 965 N. Goodman St. Call 654-4730 for appointment

•       All school age children (5 to l7 yrs) must register for enrollment in public schools

•       Bring I-94s, Landlord Statement, Verification of Immunization and any school records.

•       Obtain Enrollment Letter

 

8) Family Learning Center (FLC) 30 Hart St., Rochester, 14605 Tel #: 262-8000

•       Al l adults (l8 and over) must register for ESL classes

•       Bring I-94s and SS cards if available

•       Obtain Enrollment Letter

 

9) Medicaid/Food Stamp Office. Dept of Social Services (DSS)

•       Clients will receive notice in the mail requesting that they call to make an appointment Anchor or Vol Team Leader must call within a week to arrange appointment. The location of the DSS office will be specified on the notice.

•       Bring I-94s, Landlord Statement, enrollment letters from PLC and Placement Center, Soc. Sec. Cards (or receipts) and marriage certificate if available.

•       Only the head of the household must attend with interpreter.

 

10) Dept. of Motor Vehicles (DMV) 25 Franklin St Sibley Tower Bldg. 2nd Floor

•       All adults should obtain non-driver’s ID Card

•       Bring I-94s and Certification of ID Letter obtained from Catholic Family Center.

 

11) Selective Service/Draft Registration

•       All men ages 18 thru 25 must register

•        Forms can be obtained at Post Office or CFC office.

•       Complete form when Soc. Sec. # is available and mail to Selective Service

 

12) 90 Day Reassessment Interview at CFC 87 North Clinton Ave. l’~ Floor 262-7076

•       For all refugee cases, at least one member of the refugee family and the anchor or Vol. Team Leader must interview with CFC staff member.

•       Call CFC Case Manager to arrange appointment around refugee’s 90th  day in country.

•       Bring All DSS receipts and notices, completed and signed CFC Resettlement Checklist, Social Security Cards and any other documentation requested by Case Manager.

•       For Family Reunification (FR) cases, anchor must return both the TB Clinic and Medical Practitioner  letters signed by the clinic.

 

13) 180 Day Reassessment Interview at CFC 87 North Clinton Ave. 1st Floor 262-7076

    •       For FREE cases only if not full-time employed or if all required documentation has already been submitted to CFC.

•       Any member of the refugee family or co-sponsoring group may attend
